Your first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to our doctor. On your first visit, we will review your medical history and evaluate your podiatric health. We will perform a thorough examination of your feet and explain to you whether or not you will require any treatment.
Below is the New Patient Packet you’ll need to complete on your first visit. You can expedite matters by downloading these forms, completing them at home and handing them to our staff when you arrive for your initial visit to our office.
Please be sure to bring a photo ID, active insurance cards, and a copy of your current medication list.

We accept VISA, MasterCard, Amex and Discover credit cards, as well as cash and personal checks. We participate with most major insurance plans except TX Medicaid or its variations. Please find out with your insurance, if your plan participates with us.
Many times, people think they need a referral from another doctor or special authorization from their insurance provider to see a specialist like Dr. Saldino. However, this is not the case and most people will not need a referral or authorization.
Although many insurances do not require a referral or authorization, you may want to check with your insurance company to see if one needs to be obtained before being seen.
